Introduction and background notes (vocational context)
You are working as a health care professional in a health and social care sector where your line manager has invited you for your yearly appraisal. During the pre-assessment of your appraisal, you have been asked to create a self-assessment report of your own roles, responsibilities, experience, abilities, skills, motivation and performance as an employee. You are required to reflect on your own role in the establishment and answer the following questions below:
Note: Ensure you choose a health and social care sector and state your role when answering the questions.

The discussion surrounding your presentation revolves around a team base issue at your respective workplace. You are expected to solve the team-based issue with your guided Power Point presentation.

Presentation Guidelines:
LO3 will be a 10-minute Individual Presentation
You will apply your chosen method to solving the quality problem in your work place. The presentation lasts 10 minutes only. The presentation dates will be over a range of 2 class days, therefore it’s important that you keep within your 10 minutes’ time limit.
- You must present your work as a PPT only. Failure to do so mean you will fail the presentation.
- You should reference your work using a range of sources.
- Submit one copy (6 SLIDES PER PAGE) with your final submission on the ICON VLE.
- To achieve M2, you must apply relevant theories and techniques to your analysis in relation to team dynamics in your health and social care workplace

Case study – (Question 9-13)
The Orchardville Company is a company limited by guarantee with social firm status. It is under the umbrella of the Orchardville Society, a charity that assists young people and adults with learning Disability and Autistic Spectrum Disorder prepare for and secure employment. At present 25% of the work force have a learning disability including Mark who has started a part time job earlier this year. Mark has applied for the position of Care Assistant and a number of reasonable adjustments were made to enable Mark to accept the position.
Firstly, Mark has completed six-week work trial in order for him to decide if he liked the job and what support requirement he would need. At the end of the trial a meeting was held involving Mark, his Employment Officer and the manager to discuss the outcome of the work trial and what reasonable adjustments would be required. Eventually, they agreed that:
- The procurement of different sling as Mark found it very difficult to fix current sling to the appropriate hoist. This minor adjustment enabled mark to confidently carry out this aspect of the job to same standard of safety and at minimal risk to the employer.
- Mark also found it difficult to complete the entire task within the stipulated hours per day that was scheduled for working. The employer agreed to increase Mark hours by one hour per day to enable him to comfortably complete all the tasks and not feel under pressure. Today, Mark is still working at the Orchardville Company and being supported by the Orchardville Society Employment and Skills Service to ensure inclusion required at work.
As a manager at Orchardville, the above incidents has lead to staff training and development in understanding immediate problem solving techniques and conflict resolution. Using your modular understanding to produce a brochure showcasing competences on resolution of work based problems and problem solving initiatives.

Guidance for brochure
To produce a brochure, you need to use Microsoft Word as your chosen application
- Open a Word document.
- On the upper left, click the “File” menu.
- In the left vertical column, click the “New” tab.
- Scroll down and select ‘Brochures’ from the available templates.
- Select “Brochures” from the available templates.
- The font size should be between 12 point (minimum) and 14 point and use Arial. However, if you are providing information for elderly people or people with sight difficulties, you should always use 14 point or larger.
- Use a light background with dark print for the best contrast. • It is acceptable to use a dark background with white print (reversed out) for headings, but not for a large section of text.
- Justify the text to the left only.
- Leave space between the paragraphs and do not have too much text on the page.
- Make sure that headings are clear.

Any act of plagiarism or collusion will be seriously dealt with according to the College regulations. In this context the definition and scope of plagiarism and collusion are presented below:
Plagiarism is presenting somebody else’s work as your own. It includes copying information directly from the Web or books without referencing the material; submitting joint coursework as an individual effort.

- 5. Submission
- Initial submission of coursework to the tutors is compulsory in each unit of the course.
- Student must check their assignments on ICON VLE with plagiarism software Turnitin to make sure the similarity index for their assignment stays within the College approved level. A student can check the similarity index of their assignment three times in the Draft Assignment submission point located in the home page of the ICON VLE.
- All Final coursework must be submitted to the Final submission point into the unit (not to the Tutor). A student would be allowed to submit only once and that is the final submission.
- Any computer files generated such as program code (software), graphic files that form part of the coursework must be submitted as an attachment to the assignment with all documentation.
- Any portfolio for a unit must be submitted online.
- The student must attach a tutor’s comment in between the cover page and the answer in the case of Resubmission.
- Good practice
- Make backup of your work in different media (hard disk, memory stick etc.) to avoid distress for loss or damage of your original copy.
- Extension and Late Submission
- If you need an extension for a valid reason, you must request one using an Exceptional Extenuating Circumstances (EEC) form available from the College examination office and ICON VLE. Please note that the lecturers do not have the authority to extend the coursework deadlines and therefore do not ask them to award a coursework extension. The completed form must be accompanied by evidence such as a medical certificate in the event of you being sick.
